We would like to understand more about why people are deciding to get vaccinated against Flu or COVID-19 or a choice of one over the other.

We are asking patients across the PCN Practices (The Whickham Practice, Chainbridge Medical Practice, Glenpark Medical Centre, Teams Medical Practice and Sunniside Surgery) for their views so we can learn more.
The survey will only take a couple of minutes to complete and your personal details will not be recorded.
Your feedback will help us make improvements.
People who are eligible are usually offered an appointment for their COVID-19 or flu vaccination between October and December. You may be able to have them at the same time or you can have them separately.
