Part One in this series is highlighting the work our Primary Care Mental Health worker does for us.
Working with patients from The Whickham Practice, Chainbridge Medical Practice, Glenpark & Teams Medical Services and Sunniside Surgery, this colleague plays a crucial role in advancing integrated models of primary and community mental health care.
Their efforts aim to support adults and older adults with severe mental illnesses, helping them to thrive within their communities. The transformation of community mental health services focuses on individuals severely affected by mental illnesses, which include, but are not limited to:
- Psychosis
- Bipolar disorder
- Diagnosed personality disorders
- Eating disorders
- Severe depression
- Mental health rehabilitation needs
These conditions may often coexist with other issues such as frailty, cognitive impairment, neurodevelopmental disorders, or substance use.
